An onsen is a Japanese hot spring and the bathing facilities and inns frequently situated around them.
How to map
Draw an area 
around the grounds or the building of the onsen or place a node 
at the centre of the grounds.
Tag it with amenity=public_bath + bath:type=onsen. See also amenity=public_bath for more information.
If there is a natural hot spring map it with natural=hot_spring in addition to the bath facilities.
