This feature has been labeled as deprecated. The recommended replacement is: fixme=*.
The reason is documented in Deprecated features. You are still free to continue to use or interpret this tag as you see fit since OpenStreetMap does not have “banned features”.
Under no circumstances should you (semi-)automatically change “deprecated” tags to something else in the database on a large scale without conforming to the automated edits code of conduct. Any such change will be reverted.

The key todo=* is used to make to-do notes in the map data. For example, open ends of ways may be tagged with todo=continue, meaning the way can still be continued and it's just not mapped yet (see this tag on taginfo).

Nowadays, the key fixme=* is far more popular than todo=*. Also, fixme=* is supported by many tools such as Keep Right, JOSM, Vespucci, iD, Osmose and many other. todo=* has minimal support, usage etc.

Therefore it is recommended to use fixme=* instead.

There is also a less used variation of todo=*, which is called TODO=*, and is still used in some places.
