In some countries, it may be common to include a milestone in the address. These typically refer to "milestone" markers like those described at highway=milestone. (The "milestone" descriptor is common but not literal; often these markers will not be actual stones and will not use actual miles as the unit. Many countries use the term despite not using miles to measure distance.) The key addr:milestone=* has been proposed here: Proposal:Addr:milestone. The surrounding discussion may be a helpful reference point for looking at how these might be tagged.
Note that milestone markers may be labeled in a way which does not exactly match the actual distance along a road, but the addresses typically are based on what is written on the milestone and not an actual measurement. It follows that it is likely unhelpful to convert milestone addresses to other units. Instead, it may be a good idea to think of milestone numbers in addresses as strings of text which duplicate what is actually on the milestone. A "km 427" milestone is likely easier to find and identify on the ground if it is tagged addr:milestone=427, just like it is written.
