This tag indicates whether or not there is a ramp available. It's normally used on staircases: ways that are tagged with highway=steps. A ramp can, for example, be built next to a stairway in order to allow access to higher ground for people who use a wheelchair or have a bike. To differentiate between different types of ramps, you can use the keys from the table below.

A standalone ramp without steps can be tagged with highway=footway and incline=*.

Ramps

This is for ramps that are included in the stairway itself. Ramps that run physically separated (e.g. separated by a fence, height difference or wall), should be drawn separately.

ramp=no No ramp at all. This is the default, but it's useful to add this for completion and to aid routing for people in wheelchairs.

Presets

Ramp is included in the internal JOSM preset (since version 8011).

You can also add presets that uses the proposed scheme to JOSM. Just add the "Steps properties" preset in the JOSM presets properties (http://osmtools.de/josm/steps.xml)
