How should the nodes be added? People counters in trails are usually installed in pairs, one in each side. Should a single node be added to the way, and possibly specified with count=2? Or two nodes right next to the way? The latter doesn't reflect reality very well either in all cases, because they can be physically attached to the way (e.g. stairs).

Yes, this is a good question for such structures. Maybe making the man_made=monitoring_station as a line across the highway=* could be acceptable. Conceptually, this will look like a man_made=gantry .
As to whether it should be connected to the highway=*, for the functionality in general, man_made=monitoring_station can't explicitly show what extent it is monitoring now. For eg monitoring:water_level=* , supposedly they can be inside the natural=water drawn. However, area:highway=* is rarely found, and it's a less expected detail for highway=* . highway=speed_camera has a similar problem for relating the roadway it is regulating. It forces the use of type=enforcement .
