As an access tag

emergency=yes is used[1] to indicate that a given road is accessible to a emergency services, what may be considered as a strong hint that given road is free from physical obstacles. Note that as emergency vehicles are by definition not restricted by legal access - so for example road with highway=service + access=private may be accessible to emergency vehicles if there are no mapped physical obstacles.

For places explicitly designated for emergency access consider emergency=designated.

As a hospital property

emergency=yes is also used[2] to indicate whether a hospital (amenity=hospital or healthcare=hospital) is equipped to deal with emergencies. See also emergency=emergency_ward_entrance.
